Clinical Nasal Examination

Dr. Ece Kalafatlar Gül performs a structured rhinoplasty assessment at every consultation: external nasal profile analysis (anterior, lateral, basal views), anterior rhinoscopy and nasal airway assessment (internal nasal valve angle, Cottle manoeuvre, turbinate assessment), skin thickness evaluation, a critical determinant of result visibility and tip support assessment, identifying the cartilage structures requiring modification.

Facial Proportion Analysis

Rhinoplasty planning at DRFK uses facial thirds analysis, nasolabial angle measurement, nasofrontal angle, nasal projection relative to the upper lip, and nasal-facial width proportion assessment. Digital standardised photography is taken in the six standard views (anterior, both laterals, both three-quarters, and basal). Digital simulation may be used for patient communication with the clear understanding that simulation illustrates surgical direction, not a guaranteed pixel-perfect result.

Open Rhinoplasty

The standard approach for complex rhinoplasty is a small external columellar incision, which provides full visualisation and bimanual access to the complete nasal cartilage framework. Indicated for significant dorsal hump reduction requiring combined bone and cartilage work, complex tip reshaping, cartilage grafting, revision rhinoplasty, and post-trauma reconstruction. The columellar scar heals to imperceptibility in 3–6 months.

Closed Rhinoplasty

Internal incisions only, no external scar. Suitable for mild-to-moderate primary rhinoplasty where correction is achievable without full framework exposure. Faster recovery, less post-operative swelling, and earlier visible results. Technique is selected based on anatomy, not patient preference alone.

Preservation Rhinoplasty

The structure-first approach preserves the nasal dorsal ligaments and the natural dorsal aesthetic lines while reducing the hump by lowering the foundation beneath it (let-down technique), rather than resecting the dorsum. Produces a natural dorsal appearance, preserves the skin-framework relationship, and reduces the risk of the "operated appearance." Indicated for selected hump reduction cases where the dorsal aesthetic lines are worth preserving.

Ethnic Rhinoplasty

Rhinoplasty is adapted to the patient's ethnic nasal anatomy, addressing the specific structural characteristics of Middle Eastern, South Asian, East Asian, and African heritage noses within the aesthetic framework of each patient's cultural identity.

Revision Rhinoplasty

Septoplasty Combined with Rhinoplasty (Septorhinoplasty)

Where a breathing obstruction from septal deviation coexists with aesthetic concerns, both are corrected simultaneously. The septoplasty addresses the deviated cartilage and bone, while the rhinoplasty corrects the external form with a single recovery, dual benefit.

Open vs Closed: The Decision Matrix

Step 1: Rhinoplasty Consultation

Nasal examination, facial proportion analysis, digital photography, and digital simulation. Surgical plan agreed technique, targets, and realistic outcome discussed with full transparency.

Step 2: Pre-Operative Assessment

Blood panel, cardiovascular review, anaesthetic assessment with Dr. Zahra Ghafari (Specialist Anaesthesiologist, DRFK). Pre-operative photographs taken. Instructions on medication cessation (aspirin, NSAIDs, supplements) and fasting protocol were provided in writing.

Step 3: Surgery

Performed under general anaesthesia in DRFK's ISO-compliant theatre. Duration: 1.5–3.5 hours, depending on technique. Splint applied at wound closure.

Step 4: Immediate Recovery
