A thin laser fibre is passed inside the faulty saphenous vein. Heat seals the vein wall as the fibre is withdrawn, so the vein fibroses and is absorbed over months. Blood is redirected to healthy deep veins. EVLA is not the same as surface “laser for spider veins”, it treats the source of reflux inside the main superficial trunk, which is why long-term control is strong when the right patient is selected.
Strong candidacy is supported when duplex ultrasound shows saphenous reflux, you have symptoms or quality-of-life impact from varicose veins, and DVT in the treated limb is excluded. Pregnancy is a reason to delay. Very tortuous anatomy or post-thrombotic changes may need a different or staged plan, this is individualised in clinic.
You remain awake. The vein and treatment length are marked with ultrasound. Tumescent local anaesthetic is placed along the vein to protect surrounding tissue. A needle puncture introduces the laser fibre; as it is slowly withdrawn, laser energy closes the vein. A compression stocking is applied and you are encouraged to walk before leaving, typically 60–90 minutes on site from arrival to discharge, depending on complexity.
First week: wear compression as directed, walk as instructed, avoid hot baths, long immobility and very heavy work early on. 4–6 weeks: follow-up ultrasound often confirms closure; residual tributaries may be treated with sclerotherapy. 8–12 weeks: the sealed vein is shrinking; leg comfort and appearance usually improve. Long-term data support high closure rates for appropriately selected saphenous segments.
Bruising, tenderness, mild phlebitis-type soreness, or a temporary patch of skin numbness can happen and usually settle. Serious complications are rare when tumescent technique and walking protocols are followed.
